Schedule Appointment

If you would like to schedule an appointment with Dr. Henry at one of our offices, please call our toll free number 888-365-3436. All appointments are made through the Indianapolis office. After setting your appointment up through our front office staff, please print the forms listed to the right. These should be filled out and mailed to our Indianapolis office, along with a copy of your insurance card (front and back) prior to your scheduled appointment.

If your insurance company requires an authorization number/referral number from your primary care physician, it is your responsibility to obtain this prior to your appointment.

If you have received medical care from your referring or previous physician that Dr. Henry should review, please contact their office to request a copy to be sent to our office prior to your scheduled appointment.
